How to get services in Piute County

Most long-term supports are paid for by Medicaid, often through a waiver program. Start with Medicaid, then get on the program that fits. Where a program uses local offices, the office for Piute County is shown.

Start here Utah Medicaid · Utah Department of Health and Human Services (DHHS); eligibility by Department of Workforce Services (DWS)

Apply online (opens in a new tab) · Call 1-866-435-7414 (Salt Lake County 801-526-0950) · Apply at any Department of Workforce Services (DWS) office.

Utah does not enroll SSI recipients automatically; people on SSI must file a separate Medicaid application with DWS (Utah is an SSI criteria state).

People who are aged, blind, or disabled can apply for Medicaid, and Long Term Care Medicaid uses a separate addendum. Waivers such as Community Supports count only the person's own income for minors.

For children: None (no TEFRA option). The Medically Complex Children's Waiver counts only the child's income, but it takes applications only during open enrollment periods.

If you work: Medicaid Work Incentive (MWI): lets working people with disabilities get Medicaid by paying a monthly premium based on income. Apply through DWS.

Steps

  1. Apply online, by mail, by fax, or at a DWS office.
  2. For long-term care or waiver Medicaid, also fill out the Long Term Care Addendum.
  3. Wait for DWS to contact you by mail or phone.
  4. Send the proofs DWS asks for, such as a birth certificate.
  5. Read your decision letter and keep it.
