How to get services in Missouri
Most long-term supports are paid for by Medicaid, often through a waiver program. Start with Medicaid, then get on the program that fits. Many programs have a waiting list, so apply early.
Start here MO HealthNet (Missouri Medicaid) · Missouri Department of Social Services, Family Support Division
Apply online (opens in a new tab) · Call (855) 373-9994 · Visit your local Family Support Division Resource Center.
Missouri is a 209(b) state, so getting SSI does not give you Medicaid automatically; you must apply with the Family Support Division.
Adults who are blind or have a disability can apply under MO HealthNet for the aged, blind and disabled; if income is too high, a spend down may let you get coverage after you pay a set amount of medical costs.
For children: None (Missouri has no TEFRA option; children with developmental disabilities can use the MOCDD (Lopez) Waiver, which counts only the child's income).
If you work: Ticket to Work Health Assurance Program: lets working people with disabilities who earn too much for regular MO HealthNet keep coverage by taking part in the program, which may include a premium.
Steps
- Apply online at mydss.mo.gov, by phone at 855-373-9994, or on a paper application
- If you are 65 or older, blind, have a disability, get Social Security, or have Medicare, also fill out the Supplemental Form
- Send documents online at mydssupload.mo.gov, by mail to FSD, PO Box 2700, Jefferson City, MO 65102, by fax to 573-526-9400, or in person
- Answer any letters asking for more information
- Watch for your decision letter and MO HealthNet ID card
Have these ready
- Photo ID or other proof of identity
- Social Security numbers for everyone applying
- Proof of income such as pay stubs or benefit letters
- Bank statements and information about other assets
- Medical or disability records
